Midwifery Today accepts payment by Visa or MasterCard. Be sure to include this information with your order:

• Your account number
• The name on the credit card
• The expiration date of the credit card
• The billing address for the credit card

How to Print an Order Form

You can use your shopping basket in the Midwifery Today online store to create a printable copy of your order, and then send it to us (by postal mail of fax) or read from it when you phone in your order.

1. Create your order by adding items that you want to purchase to your shopping basket.
2. When you are finished shopping, click the Checkout button.
3. Complete the required shipping and billing information and then click Continue to Next Step.
4. Choose a shipping method. At this point you may also enter a coupon code to calculate a special sale or discount price (if any).
5. Proceed now according to your payment method:
   • If you are paying by check or money order:
     1. Disregard the payment selection, which is for Visa and MasterCard only.
     2. Click Continue to Next Step to view your order total.
   • If you are paying by Visa or MasterCard:
     1. Select the credit card type.
     2. Click Continue to Next Step.
     3. Complete the credit card information, including the name on the card, the account number, and the expiration date; or fill in the information by hand after you have printed the form.
6. Warning: Don't click Submit Order on the final ("Visa or MasterCard Information") page, because doing so would send the order over the Internet.
7. Please check that all the information in your order is correct.
8. Finally, use your browser's print function to print your order.
